This trio is specific to the anime version of this story. Dr. Gensai, "family doctor for the Kamiya dojo for 30 years," appears much later in the manga than in the TV series, and without grandchildren. He has a much more satisfying role in the TV story.

His two granddaughters, (Ayami and Tsusume) adore Kenshin and treat him like their own private plaything. He returns the adoration, basking in the love of the little girls who know nothing of assassins but rather call him "Ken nii-san." When Kenshin leaves for Kyoto many months later, their grief at his absence is just heartbreaking. The girls act as a sort of mini Greek chorus, commenting on the action. In this scene they are celebrating Yahiko's recovery from a poisoning and are actually sitting behind a huge plate of ohagi made by Megumi.
